At Elevate Wellness, your privacy is a top priority. This Privacy Policy outlines how we collect, use, protect, and disclose your personal and personal health information (PHI) in accordance with the Personal Health Information Protection Act (PHIPA) and the professional standards set by the College of Nurses of Ontario (CNO).

By accessing or using our services—whether through this website, in virtual or in-person consultations—you consent to the practices described below. Please note that prior to working with Elevate Wellness you will need to sign the Privacy Policy and the Informed Consent and Assumption of Risk and Release of Liability Agreements. 

2. What Information We Collect

We may collect the following types of information:

a) Personal Information:

  • Name, address, phone number, email address

  • Emergency contact details

  • Payment and billing information

b) Personal Health Information (PHI):

  • Medical history, medications, current care needs

  • Physical, emotional, cognitive, and social wellness concerns

  • Information relevant to senior care planning or caregiver support

We collect this information through forms, direct communication (email, phone, video), or when you book services.

3. Purpose of Collecting Your Information

Your personal and health information is collected to:

  • Provide individualized senior care planning and wellness guidance

  • Offer support for caregiver wellbeing

  • Schedule and manage appointments

  • Communicate securely with you about services

  • Maintain accurate records as required by nursing practice standards

  • Submit claims if applicable through benefit providers like GreenShield

4. How We Store and Protect Your Information

All personal and health information is stored securely using encrypted, password-protected systems. Physical records (if any) are locked and accessible only by authorized personnel. Electronic data is stored in compliance with PHIPA and industry best practices.

We do not sell, trade, or rent your personal data to third parties.

5. Your Rights Under PHIPA

As a client, you have the right to:

  • Access and request corrections to your personal health record

  • Withdraw or limit your consent (with written notice)

  • Be informed of how your data is used

  • File a complaint if you believe your rights have been violated

6. Sharing Your Information

We may share your information:

  • With your express written consent (e.g., family member, health provider)

  • If required by law (e.g., court order, mandatory reporting of abuse)

  • If there is a risk of serious harm to yourself or others

If applicable, limited information may be shared with benefit providers such as GreenShield to process service coverage claims.

7. Online Communication and Third-Party Tools

This website may use cookies or analytics tools to improve your experience. These do not access your PHI. Please note that email is used for convenience, complete privacy cannot be guaranteed over the internet.

We use secure video platforms and practice best efforts to ensure confidentiality.

8. Retention of Records

Records are retained for at least 10 years after the last contact (or 10 years after the client turns 18, whichever is longer), in accordance with Ontario regulations and CNO standards.

This website may contain links to external sites. We are not responsible for the privacy practices or content of those sites. We encourage you to read their privacy policies.

This does not include Practice Better as it is PHIPA compliant. 

10. Changes to This Policy

This Privacy Policy may be updated from time to time to reflect changes in practice, legal requirements, or technology. Updates will be posted on this page with the revised effective date.
